The Innovating care for people with multiple chronic conditions in Europe (ICARE4EU) project aimed to understand the challenges posed by their need for continuous and multidisciplinary care and to improve the design of integrated care for people with multimorbidity, helping to highlight the importance of their involvement and to make implementation more effective.

It was carried out in partnership by NIVEL, Netherlands institute for health services research;  TUB, Technical University of Berlin; UEF, University of Eastern Finland; INRCA, National Institute of Health and Science on Aging; University of Warwick; AGE Platform Europe; Eurocarers, European association working for carers.

The project was co-funded by the Health Programme 2008-2013 of the European Union.
